Laser Registration
Please submit a Laser Registration Form for all laser systems regardless of laser class. If you are using class 3b or 4 lasers, there are additional requirements that must be met.
Additional requirements for Class 3b and 4 lasers:
- Complete Laser Safety training online. (Must be completed prior to working with 3b or 4 lasers)
- Submit a training records request to obtain a copy of your training certificate.
- Submit a Laser SOP form and a Laser Application form along with copies of training certificates for the PI in charge of the laser as well as for all individuals working with 4 lasers. I f you are working with Class 3b lasers training documentation must accompany the registration. A written SOP for class 3b lasers is recommended but not required.
